Plan Facilitation Services
Plan facilitation is short term planning with a purpose. It involves the development of a person-centered plan that includes the person and the collective wisdom of the group who knows, likes and believes in the person. The emphasis is on options available to the person and not problems to “fix.” The strength-based plan identifies specific goals and steps to achieve them. Types of plans include PATH Plans, Essential Lifestyle Plans, Community Maps, and One Page Profiles.
Who is involved in Plan Facilitation?
The focus person and whoever they would like to be involved. It can be individuals who know the person well or who want to help the team think.

Various Avenues’ staff have a variety of experience with Person Centered Planning. This includes experience with PATH Plans, Essential Lifestyle Plans, Community Maps, One Page Profiles, Person Centered Descriptions, and others.
